This registration packet includes registration, medical, and agreement information required to join our dance studio.
➡️ Please note: A $50 registration fee is required to secure your dancer’s spot in class. This fee is separate from the class tuition and must be submitted along with your completed form.
Your dancer’s spot is secured only once payment and all documents are submitted in full.
